Special Requirements for Acceptance:   Must apply through PHORCAS, Graduate of ASHP accredited PGY1 residency Citizenship, Pharm.D, Licensure (preferred), Curriculum Vitae, Three letters of recommendation, Minimum GPA 3.2, One letter of reference from a PGY1 residency program director and one additional letter from a residency program preceptor. Applicants must specifically address our 24 hour, in-house, on-call program in their letter of intent.
