Residency Special Features:   

Pharmacists completing this program will be prepared to accept positions as essential members of interdisciplinary teams focused on providing patient-centered care and highly individualized symptom managements across various settings within the continuum of care and to serve as clinical faculty for a college of pharmacy. The resident will gain experience through interactions with Joint Commission Certified and award-winning palliative care and hospice interdisciplinary teams devoted to providing optimal care to patients with life-limiting illnesses. Members of the team include physicians, medical fellows in an accredited Hospice and Palliative Medicine fellowship, medical residents and students, nurse practitioners, physician assistants, nurses, social workers, massage therapists, spiritual care providers, psychologists, and volunteers. Activities include daily rounds with the robust Inpatient Palliative Care Service, twice weekly palliative care and geriatrics noon conference didactic lectures, and weekly involvement with the palliative care outpatient clinic and home hospice team serving an average daily census of over 200 patients. Additional opportunities include precepting and mentorship of PGY-1 residents and pharmacy students, participation in hospital committees, and attending two educational conferences throughout the year. The residency year may be tailored to specific areas of interest with elective opportunities offered in chemical dependency, hematology/oncology, geriatrics, emergency medicine, and critical care.

Special Requirements for Acceptance:   

Pharm.D or BS in pharmacy, satisfactory completion of an accredited PGY1 Pharmacy Residency, CV, pharmacy college transcripts, personal statement, letter of intent, three letters of recommendation, eligibility for Ohio licensure and employment at Summa Health System including background check, drug, and nicotine screening. Applications accepted through the PhORCAS system.
