Residency Special Features:   The CDPHP residency program provides a unique opportunity to engage in the advancement of the Triple Aim (patient experience, effectiveness and efficiency) with a nationally ranked health plan, recognized as an innovator in practice transformation and payment reform in the development and support of patient-centered care in medical home practices and the medical neighborhood. The resident will be involved with Medication Therapy Management and population management across all lines of business, policy and formulary management, pharmaco-economics/management and clinical pharmacy services for medical home/enhanced primary care sites. Upon completion of the program, the resident will have an extensive understanding of managed care pharmacy with enhanced clinical skills. At the completion of the residency program, the pharmacist will be a well versed participant of the health care team, in support of primary care, and able to identify the high value activities that a pharmacist can uniquely contribute and that will deliver measurable value in quality and efficiency.

Special Requirements for Acceptance:   Pharm.D.; eligible for licensure in New York; 3 letters of recommendation; letter of intent including professional goals, curriculum vitae and official transcripts.
