Residency Special Features:   *NMS Code 143813 Acute Care Track and NMS Code 143821 Ambulatory Care Track. This residency incorporates all aspects of clinical pharmacy practice through a wide range of experiences. The residency offers 4 acute care positions and 2 ambulatory care positions. CFVH is the primary site for inpatient rotations and the CFVH clinics serve as primary ambulatory care sites. Required rotations are: Internal Medicine, Ambulatory Care, Infectious Diseases (Acute Care), Geriatrics (AmCare), Drug Information, Research, Service, Teaching Certificate, and Pharmacy Administrations and Operations. Four elective rotations are selected from track specific areas: Cardiology, Oncology, Critical Care,Pediatrics, Emergency Medicine, Psychopharmacology, Nephrology, Surgery, Public Health, Women's Health, Managed Care, Home Health, Family Practice, Primary Care, and Specialty Pharmacy. The resident has multiple teaching opportunities throughout the year in a variety of didactic and clinical settings. The primary difference between the two residency tracks is in the elective choices.

Special Requirements for Acceptance:   Pharm.D; U.S. Citizenship; References - candidate must submit at least three references; two must be written by APPE preceptors.
