The Pharmacy Residency will prepare the resident for a competitive advantage in pursuing a PGY2 in ambulatory care, critical care as well as other specialty areas in the hospital. Required rotations in Emergency Department, Critical Care, Internal Medicine, etc. are complimented by elective rotations in Family Practice, Infectious Disease/Antimicrobial Stewardship, Oncology, etc. Residents are required to participate in NEOMEDs teaching certificate and gain valuable experience to strengthen their precepting skills. The resident will be given the responsibility to be the primary preceptor for an IPPE student prior to ASHP Mid-Year Clinical meeting and an APPE student afterwards. Longitudinal rotations in staffing, ambulatory clinic, teaching certificate and research project will round out the residency year.

Below are the extra requirements for PhORCAS applications that will be listed on the ASHP Residency Directory.
a. PGY1 Programs: Interested applicants must be PharmD graduates of accredited college of pharmacy, be eligible for licensure in the State of Ohio (or Florida if applicable), and submit the standardapplication materials through PhORCAS (i.e. Letter of Intent, CV, the 3 standard reference forms in PhORCAS, official college transcripts), along with:

  • 1. We request a minimum of one (two is preferred) of your three references should come from a preceptor who you have worked with in a clinical setting, related to an APPE in acute or ambulatory care. The clinical preceptor should be able to comment on your scope of responsibility, total patient load, level of autonomy, clinical abilities, and organizational and time management skills. All 3 reference writers should use the standard PhORCAS template to submit their candidate recommendation and comment on a minimum of 7 of the 13 listed candidate characteristics.
  • 2. A response to the following essay statement is required for acceptance. The essay response should be no longer than one page in length. An application will not be reviewed if this statement is missing. Please note that the essay is separate from your letter of intent. Reflecting on your APPE rotations, please provide a piece of critical feedback you have received from one of your preceptors on a direct patient care rotation. Explain how you have incorporated this feedback into your practice and/or worked to improve in this area
  • 3. Please submit all application materials via Pharmacy Online Residency Centralized Application Service (PhORCAS).

Site Special Features:   Fairview Hospital is a member of the Cleveland Clinic Health System. Fairview is the largest Community hospital on the west side of Cleveland. Fairview supports medical residencies in Internal medicine, family practice and surgery as well as a PGY2 in Emergency Medicine.  Fairview's centers of excellence are: Cardiology, Oncology, Maternity, and Emergency Medicine/Trauma II center.
