Special Requirements for Acceptance:   *NMS Code - 11831 Traditional; NMS Code - 118321 Non-traditional (current CCHMC employees only). All applicants must be eligible for licensure as a pharmacist in the state of Ohio. The following items must be received by December 25, 2018 to complete this application. Please submit the following items via PhORCAS: (1) A letter describing your reasons for applying to this program; (2) A current Curriculum Vitae; (3) Copies of official transcripts from all colleges of pharmacy attended; (4) Three letters of reference using the PhORCAS reference form; (5) Clinical Rotation Experience Document (available online at www.cincinnatichildrens.org, search "Pharmacy Residency"). In addition to PhORCAS submission, the following items must be completed: (6) A Cincinnati Children's Hospital Medical Center online application for employment. This must be submitted by December 25, 2018. (7) All candidates applying for the PGY1 Pharmacy Residency at CCHMC will also be required to complete an online preliminary interview through the Cincinnati Children's Hospital Human Resources Division. Candidates will be notified via email after complete applications are received. This online preliminary interview must be completed and submitted by 11:59 on January 1, 2019.
