Residency Special Features:   *NMS Code 190813 - PGY1 pharmacy residency. NMS Code 190821 - PGY1 pharmacy residency with a focus in mental health pharmacy. The residency provides a comprehensive year of education and experience in many areas with emphasis on providing direct pharmaceutical care. Core learning experiences include Acute Care rotations: adult internal medicine, cardiology, and critical care; Ambulatory Care Rotations: primary and specialty care clinics and Geriatric Medicine. Residents are involved in teaching pharmacy students and assisting in clinical projects. Other areas of involvement include practice management/leadership, drug information, P&T Committee, and cardiac arrest response. A wide variety of elective rotations are available in both acute care and ambulatory care settings.

Special Requirements for Acceptance:   Applicant must possess a Doctor of Pharmacy degree, US citizenship, and pharmacist licensure (or eligibility). Participation in PHORCAS (Pharmacy Online Residency Centralized Application) plus required supplemental materials as follows: a. Letter of intent: For PGY-1 residency positions, please specify which residency you would like to apply for; the PGY-1 general residency based at the Minneapolis VA Health Care System, the PGY-1residency with a focus in mental health or the PGY-1 Rural Health Care residency based at the Rice Lake, WI clinic (or all). If interested in two or more positions, please submit a separate application for each program.

c. Example of Written Work (PGY-2 Program only): Additional required supplemental material includes one (1) example of written work from PGY1 residency. Examples of written work may include a drug information response or a completed pharmacy consult (with patient identifiers removed).
d. Letters of reference: We require applicants to submit 3 references. References should be submitted through the PhORCAS Standardized Reference form.
e. Submit the required application materials through the Pharmacy Online Residency Centralized Application Service (PhORCAS) on or before the January 5th of calendar year.
