Residency Special Features:   The Kaiser Permanente San Francisco Pharmacy Department improves the health of our members and the communities we serve by providing high quality, evidenced based, and affordable pharmaceutical care. Our service is composed of 5 outpatient pharmacies, 1 inpatient pharmacy, 1 infusion center pharmacy and 250 excellent pharmacy employees. Pharmacy services in San Francisco provide services in ambulatory care, acute care, outpatient, and administration. Inpatient and outpatient pharmaceutical care services are provided 24 hours a day, 7 days a week.

The ambulatory care pharmacists practice in a variety of clinics which include anticoagulation/ direct oral anticoagulants, asthma, chronic pain/opioid management, chemical dependency recovery program, heart failure, HIV care and prevention, hospice, MTM, oncology/infusion center, pulmonary arterial hypertension, and specialty pharmacy. Our pharmacists participate in various clinical activities in multiple different settings. The outpatient pharmacists work at the height of their licensure by verifying safe and appropriate use of medications and counseling patients. The inpatient pharmacy clinical services include ICU, cardiology ICU, pediatrics, NICU, home infusion and internal medicine. Pharmacists clinical activities include but not limited to multidisciplinary rounding, medication monitoring, renal monitoring, antimicrobial stewardships, and pharmacy managed protocols. Our pharmacists work under collaborative practice agreements in these varies clinics to manage their own patients by initiating, adjusting and discontinuing medication therapy as appropriate.
