The purpose of the Kaiser Permanente Diablo Area Pharmacy Practice Residency Program is to produce a resident who is able to provide high quality pharmaceutical care in a variety of practice settings and to provide life-long contributions to the pharmacy profession.

The pharmacy practice residency is designed around the resident's professional goals and interests and will familiarize the resident with many aspects within a large healthcare maintenance organization. The program offers a broad range of experiences including primary care (some duties include: managing diabetes, hypertension, osteoporosis, lipids, COPD/asthma, dementia and Medication Therapy Management), anticoagulation, oncology (ambulatory care infusion center), pain management (inpatient), infectious disease (inpatient), HIV (ambulatory care), mental health, CHF, pharmacy management/administration, acute care (including critical care and general medicine), and drug use management. Rotations in pediatrics, transplant and cardiology are provided through affiliation with the University of California, San Francisco. Our drug information rotation is available through our affiliation with the Kaiser Regional Offices in Oakland California. Residents will also have extensive involvement in precepting and teaching pharmacy students.

Special Requirements for Acceptance:   

The applicant should be a recent graduate of an ACPE accredited school of pharmacy and eligible for California licensure by October 1st, 2018. Application materials must include: letter of intent, official transcripts, curriculum vitae, 3 letters of reference/recommendation utilizing the form in PhORCAS, and an interview.

Kaiser Permanente in the Diablo Service Area (Antioch/Walnut Creek) provides health care to over 444,000 members. Our two hospitals are located in Antioch and Walnut Creek, which are licensed for 120-beds and 233-beds respectively. In addition to the two Medical Centers, there are 8 medical office buildings in the surrounding areas including: Pleasanton, Livermore, Park Shadelands, Martinez, Antioch, and San Ramon. Our pharmacists and supportive personnel practice in inpatient, outpatient, ambulatory care and administration settings. Inpatient and outpatient pharmaceutical care services are provided 24 hours a day, seven days a week. Both of our medical centers take advantage of technology in delivering care, including CPOE, EMAR, and bedside barcoding. Pharmacists also participate in a wide range of clinical activities including primary care, anticoagulation, asthma, COPD, hepatitis, lipid management, oncology, pain management, HIV/ID, osteoporosis, dementia, drug use management, drug use education and inpatient clinical services (ID, anticoagulation, pain, NICU, ICU). Our service area provides many outstanding opportunities for pharmacists to practice in collaboration with physicians, nurses, and other health professionals under an integrated healthcare model. The Diablo Service area is also a teaching environment where students from UOP, CNUCOP, UCSF and Touro University rotate regularly.
