Residency Special Features:   *NMS: 113113 for Legacy Good Samaritan Medical Center, 113121 for Legacy Emanuel Medical Center, 113122 for Legacy Salmon Creek Medical Center, 113123 for Legacy Meridian Park Medical Center, 113124 for Legacy Mount Hood Medical Center and 113125 for Randall Children's Hospital at Legacy Emanuel. Each resident will have a home site for their required learning experiences (internal medicine, critical care, emergency medicine, administration) and their staffing requirement. Residents will be able to complete up to 2 elective rotations (12 weeks) at another Legacy Health site. Elective experiences include: ambulatory care, transplant, infectious diseases, trauma critical care, cardiovascular critical care, pediatric critical care, pediatric and adult oncology, neonatal critical care, operating room and informatics.   Longitudinal and non-clinical experiences include leadership and medication safety. Residents have the opportunity to pursue a teaching certificate through a program co-sponsored by the two local colleges of pharmacy.

Special Requirements for Acceptance:   Eligibility for Oregon and/or Washington pharmacist licensure required. Application to the program is through PhORCAS, and the application due date is January 2nd.  Additional application requirements can be found on our website.

Site Special Features:   Legacy Health is comprised of seven hospitals, primary care and specialty clinics, a research facility and a new, collaborative behavioral health center. The pharmacy departments provide decentralized patient care including collaborative drug therapy protocol management, with a strong presence in all clinical areas within Legacy.  Our pharmacy departments value a continuous learning environment, and our residency program supports this value.  Our pharmacists and residents serve as preceptors for pharmacy students from four local colleges of pharmacy.  Legacy Health is proud to train future educators, preceptors, mentors and leaders in the profession and has trained over 40 PGY1 residents since 2001.  Many of these past residents have stayed on as a member of our pharmacy team.

Legacy Emanuel Medical Center (LEMC) NMS code: 113121
Legacy Emanuel (EM) Medical Center is the largest hospital in the Legacy Health System located in Portland, Oregon. EM is home to a Level 1 trauma center, Oregon’s only burn center, highest-level care neurosciences department, a robust general surgery program and Legacy’s cardiothoracic surgery program. Legacy Emanuel is a regional leader for exceptional care in emergency medicine, critical care and complex surgical cases involving burns, cardiac, neurological, oral maxillofacial, orthopedic, trauma and vascular specialties. EM pharmacists are an integral part of the multidisciplinary teams in the 12-bed cardiovascular ICU, 24-bed neuro-trauma ICU, 16-bed burn unit, trauma recovery acute care unit, internal medicine and progressive cardiac care unit. Pharmacists also play a vital role with the infectious disease specialists, stroke team, surgical services and a busy 29-bed emergency department.
Randall Children’s Hospital at Legacy Emanuel (RCH) NMS code: 113125
Randall Children’s Hospital (RCH) at Legacy Emanuel is Legacy Health’s 165 bed pediatric hospital. RCH decentralized pharmacists are actively involved with specialized patient care, rounding daily with multidisciplinary teams. The pharmacy practice resident will have multiple opportunities to be involved in the many services offered at RCH, including a kids-only 22 bed emergency department, 38 bed level 4 neonatal intensive care unit (NICU), 24 bed pediatric intensive care unit (PICU), as well as specialties such as oncology, cardiology, inpatient rehabilitation, and general surgery. RCH is a designated level 1 trauma center with ECMO capabilities signifying RCH’s ability to care for sick and injured children from infancy through adolescence.
Legacy Mt. Hood Medical Center (LMH) NMS code: 113124
Legacy Mount Hood (MH) Medical Center, a 100-bed full-service community hospital, is the easternmost hospital in the Portland area, serving East Multnomah County along with a vast number of rural communities. The pharmacy department provides 5 decentralized clinical services: internal medicine, surgery, critical care, progressive care, and one of the busiest emergency departments in our health system with over 45,000 visits annually. Legacy Salmon Creek (SC) Medical Center is a community hospital located in Vancouver, Washington. A Pharmacy Practice Residency at Legacy’s fastest growing hospital offers dual licensure with Oregon and offers many opportunities in a robust critical care environment which includes a high volume 30+ bed Emergency Department, a 30 bed ICU/IMCU, and NICU. We also offer other services such as our engaging oncology service and medical/surgery program. SC pharmacists play an active and critical role as part of the multidisciplinary team to provide the highest quality service to our patients.

Legacy Meridian Park (MP) Medical Center is a 145-bed full-service community hospital located 13 miles from downtown Portland.  Decentralized pharmacists serve MP in a variety of settings including: a 32 bed ICU/IMCU where pharmacists round with the interdisciplinary team;  a nationally recognized acute stroke center, with pharmacists at the front line on the stroke team; a busy 27 bed Emergency Department; Internal Medicine, where pharmacists collaborate with Hospitalists throughout all phases of care; a large orthopedic surgery program; and a busy anticoagulation clinic.  The majority of our PGY1 preceptors have completed residency training.  Our location is the clinical practice site for a Pacific University School of Pharmacy practice faculty member.
